• One account must be created per user (dentist). Multiple users (dentists) cannot use a single account. Consent given via the app to anyone other than the account owner will be invalid.
• One account can be used across multiple practices. You do not have to create a separate account per practice.
• A one month notification is required for cancellation after which time you will still have unlimited access to your records and consent forms but will no longer be able to use any other functionality of the consent app.
• Your subscription will automatically renew every year unless cancelled.
• The app is completely web based and no software will be installed onto your device.
